Administrative Functions
Maintains client’s records in both office chart and home care chart
Reports any changes in client’s condition to PCP and POA as directed
Investigates any accidents or incidents immediately in accordance with policies
Investigates any member complaint or grievance and notifies the Executive Director as according to policies
Nursing Functions
Assesses member in accordance with policies
Provide basic nursing care in accordance with RN/LPN scope of practice.
Accesses client’s physical and emotional state as needed in accordance with policies
Completes comprehensive chart reviews as needed
Communicates with physicians and carries out collaborative plan or care for members
Completes necessary assessment tools
Writes and implements comprehensive care plan
Provide medication reviews and communicates with outside agencies when necessary to meet the needs of the client
Coordinates, as necessary, family meetings
Identifies community resources and communicates with outside agencies when necessary to meet the needs of the client
In the absence of support staff, the RN/LPN is able and willing to perform any of the duties delegated to the PCA, to meet the client’s needs
Qualifications and Skills:
Diploma from an accredited school of nursing (RN or LPN)
1-2 years of clinical experience in gerontology, home care, rehabilitation or acute care.
Current and unencumbered license as a Registered Nurse
Must be able to read, write and speak the English language.
Must possess the ability to make independent decisions when situations warrant such actions.
Must possess the ability to deal tactfully with clients, families, and other Extended Family staff.
Must have patience, and tact, as well as an ability to handle difficult clients.
