REGISTERED NURSE - HOME CARE

• Serve as the first clinical advisor to the home care team;.
• Oversee the care plans of all home care clients, including the development of operational, clinical, and client service protocols;.
• Conduct client assessments and intake for complex cases; monitor and analyze client data, including client history around falls, hospitalizations, and er visits;.
• Provide client consultations by phone, email, telehealth or in-person as needed, including reconciling medications, interpreting clinical information, and designing care plans to manage chronic and acute conditions in the home;.
• Monitor and triage all data captured in the home from in-home technology sources to ensure overall client safety, health and wellbeing;.
• Act as an advisor to the houseworks care coordinator/gcm, providing client assessments and reviews of care plans; and.
• Help shape the vision and strategy for a new way of delivering care in the home.
• Maintains client’s records in both office chart and home care chart.
• Reports any changes in client’s condition to pcp and poa as directed.
• Investigates any accidents or incidents immediately in accordance with policies.
• Investigates any member complaint or grievance and notifies the executive director as according to policie.
• Assesses member in accordance with policies.
• Provide basic nursing care in accordance with rn/lpn scope of practice.
• Accesses client’s physical and emotional state as needed in accordance with policies.
• Completes comprehensive chart reviews as needed.
• Communicates with physicians and carries out collaborative plan or care for members.
• Completes necessary assessment tools.
• Writes and implements comprehensive care plan.
• Provide medication reviews and communicates with outside agencies when necessary to meet the needs of the client.
• Coordinates, as necessary, family meetings.
• Identifies community resources and communicates with outside agencies when necessary to meet the needs of the client.
• In the absence of support staff, the rn/lpn is able and willing to perform any of the duties delegated to the pca, to meet the client’s needs.

• Diploma from an accredited school of nursing (rn or lpn).
• 1-2 years of clinical experience in gerontology, home care, rehabilitation or acute care.
• Current and unencumbered license as a registered nurse.
• Must be able to read, write and speak the english language.
• Must possess the ability to make independent decisions when situations warrant such actions.
• Must possess the ability to deal tactfully with clients, families, and other extended family staff.
• Must have patience, and tact, as well as an ability to handle difficult clients.

- Prolonged periods of walking, bending and climbing to care for clients.
- Must be able to stand for up to 30 minutes at a time.
- Must be able to lift 50 lbs. on a regular basis.
