Registered Nurse Case Manager

Daily Responsibilities of the RN Case Manager:

  • The Bilingual RN Case Manager will be responsible for in-person visits and telephonic follow up care throughout the greater Milwaukee area
  • Complete initial and ongoing assessments as well as care planning and service coordination
  • Re-evaluate care plans, health care needs, and revise on-going assessments
  • Provide health education and medication management
  • Clear and Active RN License in the state of Wisconsin
  • Valid Driver’s License and Insured Vehicle
  • 1+ year(s) of formal case management and experience working with the geriatric population
  • Excellent written and verbal communication skills (English/Spanish) Preferred
  • Proficient in MS Office and electronic medical records
Hours for this Position:

Full Time | Monday – Friday | 8am-5pm
