Daily Responsibilities of the RN Case Manager:
The Bilingual RN Case Manager will be responsible for in-person visits and telephonic follow up care throughout the greater Milwaukee area
Complete initial and ongoing assessments as well as care planning and service coordination
Re-evaluate care plans, health care needs, and revise on-going assessments
Provide health education and medication management
Requirements:
Clear and Active RN License in the state of Wisconsin
Valid Driver’s License and Insured Vehicle
1+ year(s) of formal case management and experience working with the geriatric population
Excellent written and verbal communication skills (English/Spanish) Preferred
Proficient in MS Office and electronic medical records
Hours for this Position:
Full Time | Monday – Friday | 8am-5pm

Salaried Compensation: $60,000 - $65,000 (highly dependent on years of relevant experience)
